Insurance and Payments

INSURANCE PARTICIPATION

Insurance companies will pay for procedures performed by our office under certain circumstances. Each insurance plan is different and we encourage you to read yours carefully. Below is a partial list of insurance plans accepted. If your insurance is not listed, you may call our billing supervisor for more detailed information. They can be reached at 269-429-7546, extension 115.

  • Medicare Part B
  • Blue Cross Blue Shield (with the exception of Blue Care Network and Blue Choice)
  • Most Lakeland Care Network policies including some CHA
Insurance carriers are rarely consistent in terms of reimbursement. They pay a portion of what they approve as being "usual and customary" for office visits, surgical procedures, treatments, and drugs. What the company approves or calls "usual and customary" is determined at its sole discretion. Therefore, all of your charges may not always be approved.

To participate with an insurance company, means that we will accept what they determine as usual and customary as payment in full. You are still responsible for any co-pay, deductible and non-approved services such as cosmetic.

We will file all Non Par and secondary insurances for your convenience. However, payment of services is required the day services are provided. Payment received from your insurance company will be reimbursed to you.

PAYMENT


Some payments are due at your office visit. These payments could include:

  • Co-payments, deductibles and coinsurance at time of service.
  • Fees for procedures that are not covered by insurance, including cosmetic or aesthetician services and skin care products
  • Payment if full for cosmetic/elective procedures
Please note: There may be additional charges posted to your account after you leave our office. These charges would be for services rendered during your visit that were not yet entered into the billing system at the time of your departure.

Additionally, in the event you are unable to keep your appointment, we require at least 24 hours notice. We reserve the right to charge a fee for missed or late-cancelled appointments. Advanced Dermatology, P.L.L.C. reserves the right to dismiss any patient that has accumulated three or more missed or late-cancelled appointments.

We accept payment in the form of cash, personal checks, VISA, MasterCard for medical charges.  All cosmetic services above $200 must be paid by credit card, unless arrangements have been made in advance.

BILLING


We will file your primary insurance claim upon verification of name, address and policy number with the health insurance company. The EOB (Explanation of Benefits) from the insurance company will list services billed, amount paid and to whom payment was made.

If an insurance payment is mailed to Advanced Dermatology, P.L.L.C. your account will be credited. If an insurance payment is erroneously sent to you, please endorse it to Advanced Dermatology, P.L.L.C. and mail to:
